About the role

Calvary TACS is looking for a dynamic RN who has a passion for working with older adults in the community setting. The aim of Calvary TACS is to avoid preventable hospital admissions and premature admission to residential aged care facilities for older adults living in the St George area. We are looking for an individual that has an understanding of holistic care and who is capable of providing case managment for clients that come through our service. This position will be well supported by CNS's in the TACS team, allied health professionals, community health NUMs and TACS coordinator.

Selection Criteria Essential: •Qualifications in Nursing with current registration with the Australian Health Practitioners Regulation Agency (AHPRA) and current unrestricted Class C driver’s licence
Essential: •Minimum of 3 years post graduate experience in an appropriate aged care, rehabilitation or community setting.
Essential: •Demonstrated high level of skills in the comprehensive assessment and management of older people with complex care needs.
Essential: •Knowledge and experience dealing with aged care, wound care, continence, and dementia issues and management of same in a community setting and/or rehabilitation setting.
Essential: •Excellent interpersonal, conflict management and time management skills, including experience working with clients from diverse cultural backgrounds
Essential: •Ability to work independently and as part of a multidisciplinary team
Essential: •Demonstrated understanding and experience in Quality Improvement
Essential: •Ability to work within the Mission and Values of Calvary Health Care Kogarah
