About the role

Lead nursing clinician for the Palliative Care Outreach Service (PCO), to support the effective and sustainable delivery of palliative care to people, their caregivers and families experiencing life-limiting illness.

 

What you bring  

Registered Nurse authorised to practise by the by the Australian Health Practitioner Regulation Agency AHPRA with demonstrated recent experience in Specialist Palliative Care. An understanding of the complexities of care for persons with advanced illness and their carers. Demonstrated ability to work effectively as a member of a multidisciplinary team with excellent communication, organisational and interpersonal skills.

Selection Criteria Essential: Registered Nurse authorised to practise by the by the Australian Health Practitioner Regulation Agency AHPRA with demonstrated recent experience in Specialist Palliative Care.
Essential: An understanding of the complexities of care for persons with advanced illness and their carers. Demonstrated ability to work effectively as a member of a multidisciplinary team with excellent communication, organisational and interpersonal skills.
Essential: Proven ability to systematically assessment and develop appropriate management plan for patients admitted to the Palliative Care Outreach service to meet patient and carer goals for care in the community setting
Essential: Demonstrated ability to co-ordinate daily allocation of patients to staff, provide leadership in response to emerging needs and delegation of tasks, ensuring equitable workload as directed by NUM or determined by agreed staffing levels
Essential: Commitment to ongoing personal and professional development
Essential: Demonstrated computer literacy
Essential: Commitment to the mission, vision and values of Calvary as it applies to the Hospital and the ability and desire to uphold these principles.
Desirable: Appropriate post-basic certificate(s) and/or experience
Desirable: Attainment Post Graduate Qualifications in Palliative Care
