About the role
As our Clinical Care Coordinator you will be responsible for the coordination of the community nursing programs across numerous streams including SA Community Care, DVA nursing and CHSP nursing during after hour periods including 4-8pm and 8am-8pm on weekends and public holidays. This position will be responsible for the coordination of clinical care for new and existing clients as well as acting as the primary point of contact for referrer's during after hour periods.


We have casual and part time opportunities available which can be negotiated by the successful candidates, ensuring we have the appropriate coverage across Monday - Friday evenings and weekends.


What you bring 

  • AHPRA registration
  • Community nursing experience with a commitment to the delivery of quality services
  • Experience coordinating service contracts and clinical programs
  • Effective communication skills with the ability to liaise with multiple internal and external stakeholders
  • Proficiency in using computer and software packages
