Calvary Health Care Bethlehem provides a range of inpatient, community and ambulatory services for palliative and progressive neurological conditions.


As a statewide tertiary provider of specialist care for people living with progressive neurological diseases (eg., Motor Neurone Disease and Huntington’s Disease), Calvary Health Care Bethlehem has a national and internationally recognized role for being at the forefront of care and research for these conditions.   With a reputation for innovation, the broad multidisciplinary clinical and research teams work collaboratively within a neuropalliative rehabilitation framework to provide best practice assessment and holistic care for both patients and families. The clinical care liaison role is varied and stimulating.


We are seeking a highly motivated individual with a demonstrated interest in person-centred care and care planning to join our Statewide Progressive Neurological Disease (PND) Service.


Our Clinical Liaisions provide comprehensive care and management advice to clients diagnosed with a PND and their families. They facilitate care planning via phone triage, needs analysis, appointment scheduling, shared care planning and liaison with members of the interdisciplinary and research teams. Clinical Liaisions within the service carry a case load and work closely with Neurologists to provide specialist care. This role also involves provision of education and secondary consultation advice to external health care providers.  Mentoring, support and training will be provided.


The successful applicant will demonstrate:

  • Highly developed interpersonal skills and professionalism
  • The ability to coordinate client care
  • The ability to identify and respond to the changing needs of individuals with a PND
  • The ability to work collaboratively and effectively within a multidisciplinary team

Prior clinical experience in working with individuals with Progressive Neurological Conditions or chronic disability is desirable, but not essential.  Applicants must be eligible for on-going entitlement to work in Australia and hold a current and valid Victorian Driver's Licence.
