Rehabilitation Patient Flow Clinical Nurse Specialist

About the role

  • Responsible for the assessment and coordination of client referrals, liaising with Rehabilitation Specialists, Nursing Unit Managers (NUMs) and Bed Managers within and outside Calvary Health Care Kogarah (CHCK) to facilitate appropriate access to inpatient and day-only rehabilitation services at CHCK.

What you bring  

  • Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia (AHPRA)
  • Current Unrestricted NSW Class C driver’s licence
  • Relevant post-graduate qualifications and at least 3 years’ experience working in the acute/sub-acute areas of rehabilitation or aged care nursing; plus satisfy the Clinical Nurse Specialist Grade 2 minimum criteria as per current Public Health System Nurses’ and Midwives’ (State) Award,
  • 100 point ID check
  • A desire to join a team committed to the care of others in a mission based organisation

Essential: • Criteria 3: Demonstrated, well-developed verbal and written communication skills, interpersonal skills, and the ability to negotiate and liaise effectively within a multidisciplinary team
Essential: • Criteria 4: Demonstrated time management and organisational skills; and demonstrated competence in the use of health-related information technology
Essential: • Criteria 5: Demonstrated advanced clinical assessment and problem solving capacity and the ability to transfer this knowledge and skill to other clinicians
Essential: • Criteria 6: Experience in working with and knowledge of the National Safety and Quality Health Service (NSQHS) Standards and Evaluation and Quality Improvement Program (EQuIP) of the Australian Council on Healthcare Standards (ACHS)
Essential: • Criteria 7: Current Unrestricted NSW Class C driver’s licence
