About the role
Calvary TACS is looking for a dynamic and flexible social worker who has a passion for working with older adults and their care networks. TACS aims to keep older adults in their home environment through time limited, slow stream rehabilitation and care service provision. We are looking for someone who has strong case management skills and a multidisciplinary mindset to complement our large and varied team.

Selection Criteria Essential: 1. Degree in Social Work and eligibility for membership of AASW
Essential: 2. Demonstrated skills in the provision of social work psychosocial assessment, intervention and case management of older people with complex needs.
Essential: 3. Experience working in Community Aged Care or Rehabilitation with a sound knowledge of community resources.
Essential: 4. Well-developed time management skills, excellent written and verbal communication and a high level of computer literacy.
Essential: 5. Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
Essential: 6. Commitment to continuous service improvement, reflective practice and service development.
Essential: 7. Current Class C drivers licence.
